Measuring Advanced Primary Care in California: Pilot Results and the Path Forward

The Purchaser Business Group on Health (PBGH)’s California Quality Collaborative (CQC) invites you to attend a webinar exploring the next step in California’s journey to scale high-quality, efficient and patient-centered primary care, also known as Advanced Primary Care.
CQC and Integrated Healthcare Association (IHA) have partnered with health plans and purchasers to pilot their Advanced Primary Care Measure Set at the practice level in California. The goal of the pilot was to assess comprehensive performance across a key set of outcomes-focused measures to understand variation and aid decision-making for system support. In this webinar we will present the results of the pilot, hear reactions from partners and explore the best next steps for the California health care system.

Rapid data exchange within the health care delivery system is integral to achieving equitable, patient-centered care for all Californians. California’s Health and Human Services Data Exchange Framework (DxF) is the first-ever, statewide data sharing agreement that will accelerate and expand the exchange of health and social services information among health care entities, government agencies and social service programs beginning in 2024.
On August 9, 2023, the Purchaser Business Group on Health/California Quality Collaborative in partnership with BluePath Health hosted a webinar to inform provider organizations about the funding opportunities available through DxF and guide them through the application process.

On Thursday, November 17, the Purchaser Business Group on Health’s California Quality Collaborative (CQC) hosted a webinar highlighting the outcomes of the CalHIVE Network, a two-year improvement collaborative focused on improving care for people with diabetes and asthma by maximizing the use of virtual care tools. Telehealth can be an essential tool to support integration for care teams as well as patients. On Wednesday, May 25, the Purchaser Business Group on Health’s California Quality Collaborative (CQC) hosted a webinar highlighting on-the-ground lessons around behavioral health integration, and understand opportunities around effective virtual care. Ellie Lopez, Director of Integrated Health Services at Borrego Health, will share Borrego’s experience integrating behavioral health into primary care, and operational examples highlighting how hybrid in-person and virtual care effectively can support integration.

CQC Webinar – Advanced Primary Care: Piloting Practice Level Measurement

On Wednesday, March 30, the Purchaser Business Group on Health’s California Quality Collaborative (CQC) hosted a webinar describing the next step in California’s journey to scale high-quality, efficient and patient-centered primary care, also known as Advanced Primary Care (APC).
The recent pandemic strain on the health system has highlighted the need for deeper transparency. This enables plans, providers and purchasers to make better informed decisions and to focus resource allocation to support providers where needed and drive better population health overall.
With this in mind, CQC and the Integrated Healthcare Association (IHA) have initiated a new APC Measurement pilot, funded initially by the California Health Care Foundation. Four purchasers, including CalPERS, Covered California and eBay, are partnering to test practice level assessment of a common APC measure set.

Physicians and provider groups in California are currently faced with immunization gaps in their pediatric population as they work to catch up with routine vaccinations deferred during COVID-19. During this California Quality Collaborative webinar, Dr. Cora Hoover from the California Department of Public Health shared statewide data trends regarding catch-up pediatric immunizations. Dr. Eric Ball, Pediatric Physician for Southern Orange County Pediatric Association, shared on-the-ground experience and lessons learned around effectively closing immunization gaps, and highlighted what small/medium practices and IPAs can do today.
